Patient & Family Education at UHN Cardiovascular Prevention and Rehabilitation Program

We offer group education sessions and workshops that provide valuable information to help people living with heart disease better manage their condition. Topics covered may include:

  • how does the heart work?
  • what is heart disease?
  • exercise safety
  • medications
  • risk factors for heart disease
  • healthy eating
  • stress management
  • guidelines for sports and leisure activities
  • how to set goals and action plans
  • how to get back on track when you return to your old habits

Our interprofessional team works closely with the team of patient education experts at the University Health Network.
